New Patients

Scheduling an Evaluation

To schedule an evaluation please call or text us at 469-573-0580 or you can email us at Info@epictherapists.com.

New Patient Forms

Once we have scheduled your appointment, we will be sending you online forms for patient information and speech/occupational therapy questionnaire along with our notices and polices.  Please take a moment to fill complete these forms prior to your appointment. If you have trouble viewing these online, you may download and fill out the forms at the bottom of the page and email them prior to your appointment to Info@epcitherapists.com.

Medical Insurance

We are happy to accept both private pay and most major insurances (listed below). For insurance coverage, we do require a doctor's referral that can be emailed to Info@epictherapists.com or faxed to 469-573-5185. There is no referral form required simply stating the requested therapy plan. No doctor's referral is required for private pay.

Payment and Financing

We accept payment with cash, check, and all major credit cards. Bills are sent via email to help reduce our carbon footprint.

Cancellation Policy

If you need to cancel or reschedule an appointment, please notify us 24 hours/ one business day before your scheduled appointment. If you don't attend your appointment or you cancel it less  than 24 hours or the same day, you will be charged a cancellation fee.
